Starting March 2, 2026
New Clinic Location
Integracare Clinics will operate out of our new clinic located at 2251 Connecticut Avenue South Sartell, MN 56377.
Closing Locations
Our current Sartell Clinic and our Country Manor Clinic will be closed. All patient appointments scheduled after March 2nd, 2026 in these two locations will take place in our new Sartell Clinic.
New Ownership in Foley
Our Foley Clinic will remain open in the same location but will operate under new ownership.
New Clinic Location
The Integracare Team will be leaving our current home on Friday, February 27th, 2026 and opening our new location on Monday March 2nd, 2026.
Our Family Practice, Physical Therapy, and Chiropractic departments will be located on the Second Floor of the building and our Lab and Radiology Department will be located on the First Floor. Elevators are available or you can take the stairs located on the right side of the entry way.
Why the Change?
We have been steadily growing and the time has come to find a space that can properly house all of our talented and compassionate team members. We are so excited to share our new space with our beloved patients so they can enjoy a state of the art facility that includes an expanded lobby, more parking, and plenty of natural light.
The new space will allow us to add even more team members and consider new service offerings. Our goal is to shorten wait times for appointments, increase access to services, and to further improve upon our patient’s healthcare experience.
While our space may be new, our approach to care is not. We remain committed to our vision of a prioritized healthcare relationship.

Effective February 27th, 2026, Integracare Clinics will close our clinics located at the following locations:

New Ownership in Foley
On March 2nd, Integracare Clinics – Foley will be operating under new ownership and with a new name – True North Family Clinic. Nurse Practitioner Shana Nelson and her husband Dan have agreed to purchase the clinic and will continue to provide Family Practice and Physical Therapy Services. Chiropractic Services will not continue in Foley.

What Changes on March 2nd?
Appointment Location
All appointments originally scheduled for our Sartell Clinic or our Country Manor Clinic will take place at our new location in Sartell.
Provider Locations
Chiropractor Danielle Granroth and Physical Therapist Laureen Bollmeyer will be moving from Foley to our new location in Sartell. Providers Jonathan Scheer, Madison Hanson, and Jennifer Lundebrek will discontinue providing care in Foley and will be working exclusively in Sartell. All patients can continue to treat with these providers by calling our office and making an appointment – 320-251-2600
Foley Under New Ownership
Our Foley Clinic will be under new ownership and will be called True North Family Clinic. Patients looking to schedule an appointment or who have questions about their care in the Foley Clinic will need to call True North Family Clinic at 320-968-7234.
In order to prevent any interruptions in care, Integracare Clinics will continue to provide billing services for True North Family Clinics. Patients who are treated at True North Family Clinic will continue to receive billing statements from Integracare Clinics and can always call the Integracare Clinics Business Office at 320-252-7800 with any questions about your bill or to make a payment.
Please note that Integracare Clinics staff will not be able to schedule appointments or answer questions regarding care provided at True North Family Clinic. Patients should contact their respective clinics directly.

The location of True North Family Clinic will remain at 471 Hwy 23 NE Foley, MN 56329.
MyChart and Health Records
You can continue to reach out to your provider via MyChart and your provider will continue to have access to your medical records. There is no need to transfer records at this time.
Insurance Network Status
Both clinics will remain in network with all major insurance carriers with the exception of UCare.
Frequently Asked Questions
You can continue to see the exact same providers you’ve seen but you just may have to see them in a different location than you’re used to. The following providers will be discontinuing care in Foley and will be exclusively treating patients in Sartell on March 2nd:

Additionally, Shana Nelson, NP will no longer be treating patients in Sartell as her practice will continue exclusively in Foley.
You do not need to request to transfer your records in order to continue treating at Integracare Clinics or at True North Family Clinic.
Yes, both clinics will continue to be accessible via MyChart.
Your bill will continue to come from Integracare Clinics even if you are treated at True North Family Clinic. For billing questions or to make a payment, you can call the Business Office at 320-252-7800.
