MyChart is a secure and free online tool that connects Integracare patients to their medical records.

If you do not have a MyChart account, but have been seen at our clinic, give us a call at 320-251-2600. We can send you a code to help you start creating your account. 

Note that Integracare uses Centracare’s MyChart database. You may be asked when creating a new MyChart account what facility you are being seen at. If Integracare is not an option, instead pick Centracare even if you are not a Centracare patient.
